Fred Wiseman's.."Titicut Follies"(USA 1967) a very heavy Doco set in a insane asylum,hard too watch at times, sad
& brutal.

A couple off the top of my head would be two Bergman films - "Through a Glass Darkly" and possibly "Winter Light" - although the latter is part of the existential angst theme of his "faith trilogy", so maybe not exactly what you're looking for. There is also a very good British picture from the mid-sixties called "Morgan" about a man who goes somewhat insane as a result of personal conflicts. There is also Truffaut's "The Story of Adele H." about delusional sexual obsession. "Repulsion" was mentioned.

sorry for not choosing from foreign ones but i do not have a huge seen-list in foreignfilms, so here is my recommendation The Pi ( Darren Aronofsky ); a great indie film as the debut of Aronofsky, it is on the top of my top20 list and it exactly fits your desire; story of mentally ill but at the same time genious Max Cohen...

'Angel Baby' (Australian) a love affair between two mentally-ill people, a moving and sad film which swept the Australian Film Institute Awards (best picture, director, and actors)

'Angels of the Universe' (Icelandic) a man developes schizophrenia after a bad love affair, very eloquent and somber

'Betty Blue' (France) a classic

'One Flew Over the Cuckoo's Nest' (American) classic Academy Award Winner

'A Beautiful Mind' (American) winner of some Academy Awards

'Girl, Interrupted' (American) Academy Award Winner

'The Caveman's Valentine' (American)

'The Saint of Ft. Washington (American)

I mention the American movies since they are also good films about mental illness.
