Tangsan (Donnie Yen, who also directs) is a young doctor in poverty-ridden Shanghai, China, in 1918. A generous soul, Tangsan welcomes all patients. When the Axe Gang wants to level his town to build a casino, Tangsan springs into action, using all of his martial arts skills to take on the gang, in the process injuring its leader. Tangsan gets more than he bargained for, however, when he begins to treat his newest patient.
